LAV Splitter Source (internal)::Audio
I get this error message on only HEVC .MP4 video files. I was running version 1544 standard and upgraded to 1646 Full Pack. Now none of these HEVC file which come from a Sony Camera will work in Media Player Classic, Windows Media Player, VLC or MPC-HC. They did work properly before the upgrade. Below is the complete error message. The VLC player error message says it can't play the file because it can't recognize an ipcm file and gives a format error. I tried changing the output in the MPC_HC player and chose all 15 options - didn't work. I selected all file formats/extensions for video and audio formats - didn't work. I deleted and reinstalled about five times - didn't work. But prior to the update these HEVC files did play in all the players and the audio worked properly. Funny thing is the audio in HEVC files work with the codecs in Premiere Pro. Just won't work in the media players. Also H.264 work in the media players just not the HEVC. I also have Micosoft HEVC extension installed. Windows 10 OS.

Re: LAV Splitter Source (internal)::Audio
Your problem is not caused by the codec pack update. VLC uses its own internal codecs and not the codecs from K-Lite.
The file contains a different audio format. But formattag 0x7069 is not a known format, so that is weird. There might be a possibility that your files are damaged/corrupted. MPC-HC should normally be able to play the video if you click away the audio error.
