Distorsion at the top of the video
I just updated to the latest BETA K-lite Codec Pack. I'm using MadVR as well.
Since the last BETA update I have some kind of lag/distorsion at the top of the screen (not whole screen) when playing videos. Can't replicate with printscreens just because the video fixes itself automatically on pause. Also in windowed mode everything seems to be alright.
I have disabled exclusive mode in madVR.
Any idea what might be the cause or fix for this?

Grab this version of MadVR:

Simply overwrite the files that were installed by the pack (any other files are not needed). That might already fix it. Resetting the settings of MadVR also often helps to fix these kind of weird issues. First test with default settings. Then adjust to your preferred settings.

Is there any difference between of MadVR from the pack and the one from their website?
I doubt it's related to the settings, I watched something this morning on an older version of the Beta pack (with MadVR active), I think from 2 weeks ago and everything was fine. After I updated with the newer version of the pack the MPC-HC started to have issues.
I tested the video files with VLC and they worked fine. So it's an issue related to the codec pack I guess.

Updated madVR and tried to reset settings.

The only difference is the version of MadVR. There was a new one yesterday.
VLC does not use MadVR.
Does the problem go away if you go back to version 13.5.5 of the pack?
Can you describe more detailed how the distortion looks like? Is it a horizontal "tear" in the video? Or does the video look blocky or blurry?

It seems it's not working even using an older package? What in the world is happening?
If I disable madVR everything works, though.
Maybe it's because of the latest Windows Insider Preview for 10? Today the Creator Update arrives so it might fix the issue.

This is the lag/distorsion/strutter issue I get when watching videos. It's happening only in that part of the screen.

That problem is called "tearing". Basically timing goes wrong and the screen displays a frame before it was fully uploaded. So you also see part of a previous frame.
Try different "VSync" settings in the graphics driver. If you have recently changed refreshrate settings, then revert to the "native" resolution and refreshrate of your screen.
I recommend to update to Fall Update since you are on Insider now. Also re-install the pack afterwards.

After asking on tenforums more about the issue, I found out it's happening only on videos that aren't 1080p.
A user claimed he has tearing at the bottom of the video for like 6 months. I've never experienced this issues. I watched last week 720p videos.
I tried all the VSync settings in Nvidia Control Panel and since it's a laptop, monitor has only one refresh rate.

Your laptop probably also has an Intel GPU. You need to update that driver as well.
There are several options in MadVR that you can try that may help:
madvr options > rendering > general > use Direct3d 11
madvr options > rendering > general > use separate device for presentation
madvr options > rendering > general > windowed overlay
madvr options > rendering > windowed mode > flush after copy to backbuffer

I changed the default graphic card back to Intel (was set to Nvidia) and now the distorsion seems to bw gone.

The screen is connected to the Intel GPU. So even when the NVIDIA one is used, the output is passed through the Intel one. This is one of the reasons why these kind of systems often have problems.
