Black Screen Video
Some of the video I have are playing only black screen with audio. The weird thing is, they play normally when in fullscreen.

It is probably a bug in madVR. Experiment with the rendering settings in madVR, like the "use separate device for" option.

You could make a debug log for the developer of madvr. Maybe he can see what is wrong.
1) Get madvr from here:
http://madshi.net/madVR.zip
2) Replace madvr.ax from the codec pack with madvr(debug).ax
3) Play a file for about 15 seconds and then close the player.
4) The should now be a log file on your desktop.
5) Zip/rar the file and upload it somewhere (like sendspace.com)
6) Replace madvr(debug).ax with normal madvr.ax file again

YES, it works. And I found the setting which causing the bug. It's the "use Direct3D 11 for presentation (Windows 7 and newer)".

That option does not bring any major benefits, so keeping it disabled is no problem. The option was added not very long ago. If it worked before, then maybe there is a NVIDIA driver bug.
