App Command is for special buttons on multimedia keyboards.
RemoteCMD is for commands send by WinLIRC or uICE.

That is short for repetition count. When you keep a button pressed on a remote control, it will repeatedly send the same command. I assume the RepCnt is the maximum number of times that MPC-HC processes the same command in sequence. But I am not 100% sure. I have never used that functionality.
