Audio delays gradually

Whenever I open a video file, it starts out in sync, after a few seconds, 5 or 10 seconds, it starts to gradually lag where the audio jumps ahead or the video lags behind and doesn't match up. It gets worse and worse. But, if I change the position of the time scroll bar, it corrects itself for a moment and then it goes out of sync again. It happens with all videos and I already tried a default reset the problem persists.
