I recommend using DXVA2 Copy-Back. Intel QuickSync does not provide any advantages.

You need to give a more detailed description of the problem. Does it use software decoding instead? Do you get a black screen?
In the settings if LAV Video decoder you can find a checkbox to enable hardware acceleration for 4K.

Previously, everything was fine somewhere in ver. 11.0.5 mpc-64bit (LAV). Software decoding not used, it does not help. Black screen no. I put dxva2 copy-back & native(writes in statistics: not using dxva) & nvcuvid, but it also does not change anything.

MPC-HC menu > Play > Filters > LAV Video decoder > click to open its settings > enable the 4K checkbox

Do you also have a ATI/AMD graphics card? That would explain why it was enabled by default in a previous version. 4K doesn't work properly on most AMD cards (due to driver bugs). But if you can confirm that MPC-HC uses the Intel GPU by default (even when using DXVA2), then the 4K option can be enabled by default again on such systems.
